Questions doctors ask
The pay questions that come up most — answered against the current contract.
How is night pay calculated for NHS resident doctors?
Hours worked between 21:00 and 07:00 attract a 37% enhancement on the basic hourly rate (1/40th of weekly salary). For an FY2 on £44,439, the basic rate is £21.35/hr, rising to £29.25/hr for nights.
What is the weekend allowance?
A flat percentage of your annual basic salary, based on how frequently you work weekends — from 3% (1-in-8 or less) to 15% (1-in-2 or more). It is not calculated per shift.
Are NHS pension contributions tax-deductible?
Yes. NHS pension contributions reduce your taxable income, so you pay less income tax. However, they do not reduce National Insurance, which is calculated on your full gross pay.
Should I opt out of the NHS pension?
Almost certainly not. While opting out saves your employee contribution (around £4,355/yr), you permanently lose the employer contribution of 23.7% of pensionable pay — money that never appears on your payslip but is entirely on top of your salary. The NHS pension is a defined benefit scheme: your eventual payout is guaranteed regardless of market performance.
How much more do locum doctors earn?
BMA recommended minimum locum rates for FY2 are £50/hr weekdays, £60/hr weekends, £70/hr nights. Working the same hours as a locum might gross ~£140,920/yr — but with no employer pension, no paid leave, and no training progression toward a consultant salary.
Can I claim tax relief on GMC fees?
Yes — the GMC registration fee (£464) is a professional expense claimable via HMRC P87 or Self Assessment. At a 20% tax rate that's £93 back per year; at 40% it's £186. You can backdate claims up to 4 years.
What does LTFT mean for my pay?
Less Than Full Time (LTFT) training means all pay components — basic salary, additional hours, nights, weekend allowance, on-call, FPP, and London weighting — are pro-rated to your agreed LTFT percentage. Annual leave and study leave are also pro-rated, and NHS pension protections remain in place.
How much annual leave do resident doctors get?
Under the 2016 TCS: 27 days if you have less than 5 years NHS service, 29 days for 5–9 years, and 33 days for 10+ years. Plus 8 bank holidays. Study leave is up to 30 days per year (pro-rated for LTFT).
